Reporting: Data Quality - How to Correct UDE Errors Flagged on the Data Completeness Report
The information provided by the HMIS Data Completeness Report is intended to help you make corrections to client HMIS records within Community Services. Please contact the AKHMIS Help Desk for the fastest response to any questions.
Universal Data Elements (UDEs)
DNC = Stands for a Data Not Collected response
DKR = Stands for a Client Doesn't Know or Client Refused response
These flags are scored on ALL ENROLLMENTS

Date of Birth Flags
The Date of Birth ( DoB) and DoB Data Quality (DoB DQ) fields are combined into one column with red font flagging records with missing information and other colors flagging mismatches between this enrollment and previous enrollments in the clients record.
Red Font/Error flag: (No point) Enter information, if known
▪ MISSING DoB is missing
▪ MISSING or ##/##/#### DoB DQ is missing or DNC (regardless of DoB answer)
▪ ##/##/#### DoB DQ is Client refused or Client doesn't know and DoB is not blank
▪ ##/##/#### DoB DQ is Partial DoB and DoB is not 1/1/yyyy
Other Colored Text/Warning Flags: (Still receive point) Verify accuracy of any/all other DoBs in client record and edit with appropriate, accurate information, if known
▪ ##/##/#### DoB on entry is null, but client record now has a DoB filled out
▪ ##/##/#### DoB is filled out, but DoB DQ is Client refused or Client doesn't know
▪ ##/##/#### DoB on entry does not match most recent DoB in client record
▪ ##/##/#### DoB on entry matches most recent DoB in client record, but there are additional DoBs on client record
▪ ##/##/#### DoB on or after Start Date (a client cannot be in a project before birth, and it is unlikely a client entered the project the day they were born)
▪ ##/##/#### DoB before 1/1/1935 (likely incorrect, flagged for review - but may be correct)
