Data Elements: 4.05 - 4.10 Disabling Conditions

Data Element

Each client self-reports (adults may provide answers for children in the household) if they have a disabling condition and if there is indication that the disabling condition is expected to be of long continued and indefinite duration and substantially impair the client's ability to live independently.

Disabling conditions include:

  1. 4.05 Physical (HUD)
  2. 4.06 Developmental (HUD)
  3. 4.07 Chronic Health Condition (HUD)
  4. 4.08 HIV/AIDS (HUD)
  5. 4.09 Mental Health Disorder (HUD)
  6. 4.10 Substance Use Disorder (HUD)

o   Alcohol use disorder (HUD)

o   Drug use disorder (HUD)

o   Both Alcohol and Drug use disorder (HUD)

   

Data Element Type

4.05 - 4.10 Disabling Conditions are Common Program Specific Data Elements.

Collection Point

4.05 Disabling Condition records are created at project start, update, and project exit to reflect the information as of the date of each phase of data collection. Disabling Condition update records should be created at any time during a project stay if a client's disability status changes.

Why It Matters

Disabling Condition is used to determine whether a client can access Permanent Supportive Housing projects.

Data Quality

The Data Quality Performance Scorecard looks at the following and flags as an error if:

  • Gateway response is missing, mismatch or Data Not Collected.
  • Type entries are missing, there are multiple open entries for the same type, or response is Data Not Collected, Determination is blank, or Unidentified.

How to correct: Enter value, if known on the Project Entry, Project Exit, or Interim Assessment.
