WHY COLLECT DISABLING CONDITIONS INFORMATION? 

Collecting information about Disabling Conditions helps providers understand if clients have any disabling special needs which contribute to their experience of homelessness or may be a factor in housing. Does a client have a disabling condition that affects their ability to access housing? If the answer is yes, the client may be more eligible for some housing and services.  

  

HUD AND AK DISABLING CONDITIONS 

The Disabling Conditions question is based on HUDs definition of disabling conditions. HUDs definition includes the requirement that the disabling condition is  long term, of indefinite duration, and impacts the clients ability to live independently. This field should be answered based on information the client disclosed at the time of intake. If a client does not disclose a disabling condition but in your professional experience you believe they have a disabling condition, you can overwrite their no answer with a yes. This is a client-centered approach because it helps document the clients eligibility for some resources.
