Data Elements: V2 Services Provided - SSVF

Data Element

The provider records each service on the date it is provided. Services should be attached only to the head of household’s record. Do not create duplicate records of a single service for each household member.

Data Element Type

V2 Services Provided – SSVF is a U.S. Department of Veteran Affairs (VA) Program Specific Data Element and collection is required for SSVF Homelessness Prevention and PH – Rapid Re-Housing projects.

Collection Point

Occurrence point (as service is provided). Services should be attached only to the head of household’s record. This element is required for SSVF RRH (Rapid Re Housing) & HP (Homelessness Prevention) projects and is optional for other VA projects.

Why it Matters

For planning and reporting purposes, it is critical for grantees to collect and report services data via HMIS data entry on a timely basis.
