Data Element

Client self-reports their health insurance coverage. Sources include:

  1. Medicaid
  2. Medicare   
  3. State Children's Health Insurance Program    
  4. Veteran's Administration (VA) Medical Services    
  5. Employer - Provided Health Insurance    
  6. Health Insurance obtained through COBRA    
  7. Private Pay Health Insurance    
  8. State Health Insurance for Adults   
  9. Indian Health Services Program    
  10. Other

Data Element Type

4.04 Health Insurance is a Common Program Specific Data Element.

Collection Point

4.04 Health Insurance data is collected at project start and project exit. A Health Insurance record must be created at any time during a project stay if health insurance coverage information changes.

Why It Matters

This information is used to determine whether clients are accessing all mainstream medical assistance benefits for which they may be eligible, and to ascertain a more complete picture of changes to economic circumstances between the project start and project exit.

Data Quality

The Data Quality Performance Scorecard looks at the following and flags as an error if:

  • Gateway response is missing, mismatch or Data Not Collected.
  • Type entries are missing, there are multiple open entries for the same type, or response is Data Not Collected, Covered Y/N is blank, or Unidentified.

To correct: enter value, if known on the Project Entry Assessment, Project Exit Assessment, or Interim Assessment.
