HMIS 101: When To Complete And Enter An Annual Assessment Into AKHMIS

What is an Annual Assessment?

An Annual Assessment is a snapshot of how the client is doing in the program. The Annual Assessments are completed by providers that are required to collect the following data elements: Income and Sources, Non-Cash Benefits, and Health Insurance.

  

Who receives an Annual Assessment?

An Annual Assessment must be created for clients/households who are enrolled in certain project types for a year and longer. The Annual Assessment must include updating both the Head of Household's record and any other household members at the same time. 

  

When does an Annual Assessment need to be completed?

To be considered completed, the Annual Assessment must be recorded in AKHMIS within the period of 30 days before or 30 days after the annual anniversary of the client's entry into the project, regardless of the date of the most recent update or any other assessment.

 

How to identify when a client needs an Annual Assessment?

The easiest way to determine when a client needs an annual assessment is to run the Data Completeness Annual Assessments report in BusinessObjects.

 

What information is collected for the Annual Assessment?

Depending on the project's funding source, you may see additional assessment data collection criteria. Below are the minimum data collection requirements from the 2024 HMIS Data Dictionary.

  • 4.02 Income and Sources
  • 4.03 Non-Cash Benefits
  • 4.04 Health Insurance
