C2 Moving On Assistance Provided is part of the strategic priority to end homelessness, HUD encourages communities to explore Moving On strategies in their communities for clients in PSH (Permanent Supportive Housing) who may no longer need or want the intensive services offered but continue to need assistance in maintaining their housing.

The Moving On (C2) data element is to be collected as a service for the Head of Household at any point within the project enrollment that the information changes.

Options

  • Subsidized housing application assistance
  • Financial assistance for Moving On (e.g., security deposit, moving expenses)
  • Non-financial assistance for Moving On (e.g., housing navigation, transition support)
  • Housing referral/placement Other (please specify)

  • HUD: CoC – Collection required for PH-Permanent Supportive Housing (disability required for entry)
  • HUD: Unsheltered Special NOFO – Collection required for PH-Permanent Supportive Housing (disability required for entry)
  • HUD: Rural Special NOFO – Collection required for PH-Permanent Supportive Housing (disability required for entry)
